Has anyone done a communication engagement / effectiveness type of survey?

We’re re-vamping our sharepoint site and using a new email tracking platform, along with an employee app. We’re thinking of doing a survey to see if people are receiving the information they need in the most efficient medium. If anyone has suggestions for questions that we could use as baseline measurements to track over time, or specific process types of questions related to communication that would be great!
