I’ve found that this simple template has worked well for organisations in the past. I’ve pasted in the text as I cannot upload a PDF. I’ve found a cadence of updating a Personal Development Plan each quarter or twice per year to work well.

What you’re talking about could also be referred to as setting developmental goals. I found this article recently really useful for discussing that. It’s not a template (@Clara_McCormack’s post is great for that) but has some info on philosophy and approach that I got a lot of value from.
